This Notice of Privacy Practices describes how Dunnellon Dentistry may use and disclose your protected health information (“PHI”) and how you can access that information. We are required by law to maintain the privacy of your PHI and to provide you with this notice.

Treatment: To provide, coordinate, or manage your dental care.
Payment: To obtain reimbursement for dental services provided.
